Am I a good fit for the Collaborative Change Leadership (CCL) Program?

The CCL Program is an interprofessional program and is aimed at leaders from all areas of the health care system. You must have 5 years of experience in a leadership role and be driven to engage communities to create as well as sustain system change to improve health care access for underserved communities. What are the requirements for participating in the CCL program?

To participate in the CCL Program you must have 5 years in a leadership role in a health care or community organization. You will need an identified Capstone Project which engages an underserved community to create system change to better serve that community. For this process you will need the support of your organization and a team member registered in the program from your organization to work with. How do I register for the CCL Program?

To register for the CCL Program you must submit an abstract to info@collaborativechangeleadership.ca with names, email contacts and roles of colleagues from their organization and community expecting to attend the program (a minimum of 2 participants per organization required).You should attach the following supporting documents:
• curriculum vitae
• motivation letter
• one page project outline
• letter or support from the healthcare or community organization

What type of project will I be working on?

Learners in the CCL Program will co-create a capstone project that will be implemented in their organization. This project will engage communities in a meaningful way and create sustainable system change that will improve healthcare access to an underserved group. These groups include but are not limited to the frail elderly, aboriginal peoples, mental health, non-communicable diseases/ chronic illness, youth and women, and people of lower socio-economic status. The focus is on, but is not limited to, interprofessional care and education, quality and safety, and patient/family/community-centered care.

What are the travel and time commitments of the program?

The 2014-2015 cohort of the CCL Program is located in Toronto, Ontario, Canada, and will require attendance of all five two-day face-to-face sessions that will occur over a ten month period. There will be an additional time commitment required for online learning and coaching as well as work on the Capstone Project.
